Maintaining patients' medical records is perhaps as important as providing the best possible treatment to them. This case aptly demonstrates why it is so…

Jameel, a septuagenarian, was suffering from multiple complications such as DM II, HTN, p/w ACCHTN, LVF, CKD, Hyperkalaemia, Thrombocytopenia and was a known hypertensive. He was rushed to Jayadeva Institute where the doctors immediately admitted him to the ICU and started the treatment. Around midnight Jameel's relatives panicked seeing that his condition was not improving. Surprisingly, they took discharge against medical advice and admitted him to Fortis.
